You will receive a confirmation email when your report has been submitted.

Q1: July 1 – Sept 30 (Due Oct 15)
Q2: Oct 1 – Dec 31 (Due Jan 15)
Q3: Jan 1 – Mar 31 (Due April 15)
Q4: April 1 – June 30 (Due July 15)

Completed forms must be received by the Brain Injury Alliance no later than midnight on the due date.

1st quarter statistics: Due October 15 (reporting on July 1 – Sept 30)

2nd quarter statistics: Due January 15 (reporting on October 1 – Dec 31)

3rd quarter statistics: Due April 15 (reporting on Jan 1 – Mar 31)

4th quarter statistics: Due July 15 (reporting on April 1 – June 30)
