Quarterly statistics:

1st quarter statistics: Due October 15 (reporting on July 1 – Sept 30)

2nd quarter statistics: Due January 15 (reporting on October 1 – Dec 31)

3rd quarter statistics: Due April 15 (reporting on Jan 1 – Mar 31)

4th quarter statistics: Due July 15 (reporting on April 1 – June 30)

Community program grant reports:

Interim report form: Due January 31

Final report form: Due Sept 30
(Final report period – July 1 to June 30)

Training grant reports:

Training grant report form: Due July 15
(for training from October 1 to June 30)
Statistical, impact information and stories will be required

Gur Singh Education Reports:

Gur Singh organization grant report form: Due July 15

Scanned or photocopied forms will not be accepted.

Scanned supporting documents are acceptable.

Digital signatures are required in the forms.
